How do I make an appointment?

If your doctor feels you should have further treatment for your condition, and if you choose to see us, then your doctor will help by arranging a referral.

If you are referred through Choose and Book please call us on the number on the handout your doctor gives you.

If we receive your referral by letter or fax we will attempt to contact you by phone to make an appointment. If after two days we are unable to reach you we will write to you, inviting you to contact us.

If you have not heard from us within 4 days of being referred please call our NHS administrator on 01256 442580.
